As parents, we have a tremendous responsibility to train and mold our children in a way pleasing to Him.  Thus, we firmly believe God has called Northside Baptist Christian School into existence to assist you in this commitment.   Please read our article about Why Christian Education Doesn't Cost, it Pays!

Below is a list of forms that need to be completed and returned to Northside Baptist Christian School in order for your child to be considered for admission.

New Student Application

Covenant Agreement

Financial Commitment/Insurance Waiver

Medical Treatment Release Form

     (The Medical Treatment Release Form must be notarized)

Please Also submit the following

Health Department Requirements

Kindergarten 4 and All New Students

1. Birth Certificate from Bureau of Vital Statistics
2. Current Kentucky Immunization Certificate
3. Complete physical exam from a physician completed within one year of entering kindergarten
4. Social Security Number
5. Varicella Immunization (Chicken Pox)
6. Eye exam by optometrist or ophthalmologist

Additional Requirements for Kindergarten 5 and All New Students

1. 3 doses of Hepatitis B and 2 doses of MMR are required at Kindergarten 5 entry

Additional Requirements for Sixth Graders

1. Complete physical exam from a physician (yellow form) 

If your child attended Kindergarten 4 at Northside, you will need to bring a new KY immunization certificate showing proof of the Hepatitis and MMR vaccines.  The remainder of the information should already be on file in the office.
